Our comprehensive home health care services are designed to meet the unique needs of each patient, from skilled nursing and personal care to therapy and emotional support. With a team of experienced Registered Nurses, certified therapists, and compassionate caregivers, we provide specialized services such as wound care, IV therapy, physical and occupational rehabilitation, speech therapy, and home health aide assistance. We also partner with Medical Social Workers to guide families through life’s challenges, offering counseling and access to community resources. Skilled Nursing

Our Registered Nurses can provide a wide range of care services at home.  Our nurses have extensive experience. For patients who have complicated wounds below the knee, we collaborate with Podiatry Specialty Physicians. Below is a list of some of the services included in this program:

Home Health Aide

Under the supervision of a registered nurse, a Home Health Aide is responsible for taking care of a patient at home. Duties such as the following are included in this program:

Physical Therapy

Physical Therapists assess patient’s ability to walk, need for assistive devices, functional strength, endurance and safety.

Occupational Therapy

Occupational Therapist evaluate the patient’s ability to perform Activities of Daily Living (ADLs), patient’s safety at home and identifies challenges that prevent the patient from being independent.  The Occupational Therapist then develops a program to address these issues in order to reach the patient’s optimal functional level.  The Occupational Therapy program will address areas such as poor balance, decrease functional strength, decrease coordination, and/or an inability to reach or bend.

Medical Social Services

During troubling times, the support and guidance of a Medical Social Worker can make a big difference. When you find out that someone you love is chronically ill or passes away, you need all the assistance you can get. At times, we also handle problems in the household that can be addressed with proper parenting or access to community resources.

Medical Social Services can provide you and your family with:

  • Assist the patient, family or caregiver in the resolution of social or emotional problems through the assessment of community and or financial resources.
  • Provide counseling related to relationships and or personal matters on a short term basis.
